What Is Tummy Tuck Surgery?

"A tummy tuck does more than improve appearance," explains Dr. William Umansky. "Many patients tell us they finally feel comfortable in their clothes again, and some notice improvements in posture and core strength after we repair their abdominal muscles."

A tummy tuck, medically known as abdominoplasty, is a plastic surgery procedure that removes loose skin and excess fat from the abdomen while often also tightening the underlying muscles. The result is a flatter, firmer abdomen with improved contours.
Unlike liposuction, which targets only fat, a tummy tuck addresses multiple concerns simultaneously. Most importantly, it removes loose skin that liposuction alone cannot address. The procedure can also repair separated abdominal muscles (a condition called diastasis recti) and laxity of the abdominal wall musculature, which can contribute to an unflattering silhouette from bulging or protrusion of the abdomen.

Common Reasons for Tummy Tuck Surgery

Patients at our La Jolla practice seek tummy tuck surgery for a variety of reasons. Understanding what this procedure can address will help you determine if it aligns with your concerns.

Post-Pregnancy Changes

Pregnancy places extraordinary demands on the abdominal wall. The skin often stretches significantly, the rectus muscles often separate along the midline, and fat can accumulate to support the growing baby. After childbirth, many women find that their abdomen never fully returns to its pre-pregnancy state, regardless of how much they exercise or how carefully they eat. A tummy tuck can repair separated muscles and remove the stretched skin that creates a persistent belly “pooch.”

Significant Weight Loss

Whether achieved through diet, exercise, GLP-1 medication or bariatric surgery, major weight loss is a remarkable accomplishment. However, skin that has been stretched for years often lacks the elasticity to contract down around a smaller body. The result can be excess skin that hangs from the abdomen, creating discomfort and making it difficult to find clothes that fit properly. A tummy tuck removes this redundant tissue, allowing patients to truly appreciate the results of their weight loss efforts.

Aging and Loss of Skin Elasticity

Even without pregnancy or dramatic weight changes, the abdomen can change with age. Skin naturally loses collagen and elastin over time, leading to laxity and sagging. Fat distribution shifts, and the muscles may weaken. A tummy tuck addresses these age-related changes to restore a more youthful abdominal profile.

Diastasis Recti

This condition occurs when the two sides of the rectus abdominis muscle separate along the midline of the abdomen. While most commonly associated with pregnancy, diastasis recti can also result from repeated weight fluctuations or improper exercise technique. The separation creates a bulging appearance and can contribute to lower back pain and poor posture. Tummy tuck surgery includes the repair of this muscle separation with internal sutures.

Previous Abdominal Surgery

Some patients have scarring or contour irregularities from previous surgeries, including cesarean sections. A tummy tuck can often improve the appearance of old scars while addressing other abdominal concerns.

Am I a Candidate for Tummy Tuck Surgery in San Diego?

"I spend considerable time during consultations discussing timing," notes Dr. Jeffrey Umansky. "A tummy tuck is an investment in yourself, and we want to optimize the conditions for your results to last. If waiting makes sense for your situation, we'll tell you that honestly."

Good candidates for tummy tuck surgery generally share the following characteristics:

  • Have loose or excess abdominal skin that does not respond to diet and exercise
  • Are at or near a stable weight they can maintain
  • Be in good overall health without medical conditions that could impair healing
  • Do not use nicotine products or are willing to quit at least four weeks before and after surgery
  • Have realistic expectations about what abdominoplasty can accomplish

Women who plan to have more children are typically advised to postpone tummy tuck surgery. Although it is safe to have a pregnancy after an abdomioplasty, a subsequent pregnancy can stretch the repaired muscles and skin, potentially reversing the improvements achieved through surgery.

Similarly, patients planning significant weight loss should reach their goal weight before considering a tummy tuck. While the procedure removes fat cells permanently, substantial weight gain after surgery can affect your results.

Types of Tummy Tuck Procedures

At Umansky Plastic Surgery, we offer different tummy tuck techniques to address varying degrees of concern. Your plastic surgeon will recommend the approach best suited to your abdominal anatomy and personal goals.

Full Tummy Tuck

A full abdominoplasty addresses the full length of the abdominal area, from the lower ribcage to the pubic region. This procedure is appropriate for patients with significant skin laxity, muscle separation, and excess tissue above and below the navel.
The incision extends horizontally across the lower abdomen and is typically placed low enough to be concealed by underwear or a bikini bottom. A second incision is made around the navel (umbilicus) so that it can be repositioned after the skin is tightened. Most tummy tuck patients at our practice choose this comprehensive approach.

Mini Tummy Tuck

A mini abdominoplasty is a less extensive surgery that targets concerns isolated to the area below the belly button. The incision is often shorter than in a full tummy tuck, and the navel does not need to be repositioned.
This option works well for patients with mild skin laxity and minimal muscle separation limited to the lower abdomen. Recovery time for a mini tummy tuck is generally shorter compared to a full procedure.

Extended Tummy Tuck

For patients with significant excess skin extending to the flanks and lower back, an extended tummy tuck may be recommended. The incision is longer, wrapping around the hips to address skin laxity on the sides of the body. This technique is often appropriate for patients who have lost substantial weight or who carry excess tissue beyond the front of the abdomen.

“We determine which technique is appropriate based on your anatomy and what you want to achieve,” explains Dr. William Umansky. “The consultation allows us to evaluate your skin quality, muscle tone and fat distribution so we can recommend the right approach.”

What to Expect During Tummy Tuck Surgery

Tummy tuck procedures are performed at our accredited surgical facility in La Jolla under general anesthesia administered by a board-certified anesthesiologist. The surgery generally takes two to four hours, depending on the extent of correction needed and whether additional procedures are being performed simultaneously.

Tummy Tuck Incisions

Dr. Umansky makes a horizontal incision along the lower abdomen, usually positioned just above the pubic area. The incision is usually placed below any existing cesarean section scars, which are removed in the process. The incision may be shorter for a mini tummy tuck or extend from hip to hip, with the exact length determined by how much skin needs to be removed. An additional incision is made around the navel for full and extended tummy tucks.

Muscle Repair

Through the surgical exposure, your surgeon accesses the underlying abdominal muscles. If diastasis recti is present or there is abdominal wall laxity, the muscles are tightened at the midline and sutured into position. This internal repair creates a firmer abdominal wall, reduces the appearance of a protruding belly, and can also improve core strength.

Skin Removal and Repositioning

Once the muscles are tightened, the skin is pulled downward and redraped over the abdomen to create a smoother, tighter contour. A new opening is created for the navel, which is sutured into its natural position. Excess skin is removed from the lower abdomen.

Closure

Drs. Umansky frequently use a drainless technique when performing abdominoplasty, which eliminates the need for surgical drains and simplifies the recovery process. Incisions are closed with multiple layers of sutures using meticulous technique to promote optimal healing and minimize visible scarring.

Tummy Tuck Recovery and Aftercare

Following surgery, you will be fitted with a compression garment that reduces swelling and supports your newly contoured abdomen. Most patients experience moderate discomfort during the initial recovery period, which can be managed effectively with prescribed pain medication. Many patients find they can transition to over-the-counter pain relievers within three to five days.

The First Two Weeks

Plan to spend the first several days recovering at home. Walking in a slightly bent-over position during this time often feels better as your body adjusts to the tighter abdominal skin. This is normal and will improve gradually. Walking is encouraged starting the evening of your surgery, as movement promotes circulation and reduces the risk of blood clots. Avoid lifting anything heavier than 10 pounds during this initial phase.
Bruising and swelling are common and will begin to subside within the first week or two. However, some swelling may persist for several months before your final results become fully visible.

Returning to Normal Activities

Most patients return to desk jobs and light activities within seven to 14 days. Strenuous exercise and activities that engage the core muscles should be avoided for approximately six weeks. Your tummy tuck surgeon will provide specific guidance based on your individual healing progress.

Scar Management

Your incision will appear red at first and may take six to 12 months to fade significantly. Drs. Umansky recommend topical silicone gel and other scar care protocols to promote optimal healing. In some cases, laser treatments can further improve the appearance of scars.
“Patients often tell us the recovery was more manageable than they anticipated,” notes Dr. Jeffrey Umansky. “Following our aftercare instructions makes a real difference in how smoothly the healing process goes.”

Tummy Tuck Results

You will notice an immediate improvement in your abdominal firmness and definition after surgery, though swelling will obscure the final results for several weeks. Most patients see their abdominal contours continue to improve over three to six months as residual swelling resolves and the adjusted tissues settle into their new position. Your incision scar will continue to fade for up to a year.

How Long Does a Tummy Tuck Last?

The improvements achieved through tummy tuck surgery are designed to be long lasting. The skin and fat removed during the procedure are permanently gone, and the muscle repair remains intact. Maintaining a healthy lifestyle is the most important factor in preserving your results. While the removed fat cells cannot return, significant weight gain can cause the remaining fat cells to expand and may stretch your skin. A future pregnancy can also affect your results by stretching the abdominal muscles and skin.

“Most of our patients maintain their results for many years, if not decades,” says Dr. William Umansky. “The key is approaching this surgery at the right time in your life and committing to healthy habits afterward.”

Frequently Asked Questions About Tummy Tuck Surgery

How Much Does a Tummy Tuck Cost in San Diego?

The cost of your tummy tuck depends on whether you need a mini or full abdominoplasty, whether additional procedures are included, and other factors specific to your surgical plan. During your consultation, we will provide a detailed tummy tuck cost breakdown so you understand exactly what to expect. We also offer financing options to help make the procedure more affordable.

Will I Have Visible Scars After a Tummy Tuck?

Tummy tuck surgery does leave a scar, but it is strategically placed low on the abdomen where it can be concealed by swimwear or underwear. The scar will appear red initially but typically fades significantly over six to 12 months. Our surgeons provide detailed scar care instructions and can recommend treatments to optimize healing.

Can I Get Pregnant After a Tummy Tuck?

Getting pregnant after abdominoplasty is possible, but it can compromise your results. The stretching of skin and abdominal muscles during pregnancy may reverse the improvements achieved through surgery. For this reason, we recommend that patients consider their pregnancy plans and timing before undergoing abdominoplasty.

What Is the Difference Between Tummy Tuck Surgery and Liposuction?

Liposuction removes fat through small incisions using a suction device, but it cannot address loose skin or repair separated muscles. A tummy tuck removes excess skin, eliminates fat from the lower abdomen, and tightens separated abdominal muscles. Many patients benefit from combining these procedures for more comprehensive results and more harmonious body proportions.

Can I Combine Abdominoplasty With Other Procedures?

Yes. Patients often choose to address multiple concerns in a single surgery. Combining plastic surgery procedures reduces overall recovery time compared to having separate operations and can provide more comprehensive results. The cost is also reduced compared to separate procedures, as the facility fee, anesthesia fee and many other related costs are only charged once for the single combination surgery.

A mommy makeover combines tummy tuck surgery with breast procedures such as breast augmentation, breast lift or breast reduction to address changes that occur after pregnancy and breastfeeding. Liposuction or Smartlipo is frequently performed alongside a tummy tuck to contour the waist, hips and flanks. Fat removed during liposuction can be transferred to the buttocks through a Brazilian butt lift, adding buttock volume and projection while eliminating unwanted fat from other areas.
